It seems this is a 6th grade assignment is from a school in Tupelo Mississippi.
In it, students are asked a battery of questions each answer contains a (D) or an (R) next to it. Ultimately students have their answers tallied up by the teacher and are graded as either (D) or (R).
Students were instructed by the teacher not to tell their parents about the assignment so naturally all of them did.
